The effectiveness evaluation is the step in which it is verified if the actions implemented in Step 5 should be critically analyzed with the objective of evaluating if the measures taken were effective or not.


To evaluate the effectiveness of the actions, locate the pending task on your tasks screen, which will be in the Effectiveness tab:



When clicking on the pending task, the system will keep the entire flow of the NCR for the evaluation, as detailed:

  • Step 1 – Details: description of the nonconformity and general data;

  • Step 2 – Correction: emergency actions adopted;

  • Step 3 – Cause: analysis and identification of the root cause;

  • Step 4 – Planning: action plan to eliminate the cause;

  • Step 5 – Implementation: execution of the actions and proofs.


Did the Implemented Actions Eliminate the Causes of the Nonconformity?

This question is crucial to determine if the implemented corrective actions were effective in eliminating the root cause of the nonconformity.


Effective evaluation:


If the answer is Yes, this means that the implemented actions managed to resolve the problem definitively. When clicking on Yes, the system will proceed to the next evaluation steps.


Evidence field: This field is mandatory and it will be necessary to detail the evidence that proves that the causes of the nonconformity were in fact eliminated.



Insert attachments: This field is not mandatory, but it allows including documents, reports, or records that show that the corrective actions were implemented successfully and that the problem was resolved. 



Evaluation of Risks and Opportunities: The next step will be to evaluate if it is necessary to update the risks and opportunities related to the nonconformity. Even if the cause has been eliminated, it is important to verify if any new risk has arisen or if the opportunity for improvements can be taken advantage of. 


In the "Observation" field, objectively write details about the need. 

Changes in the Quality Management System: The final step will be to verify if the implemented actions require any change in the quality management system. If the answer is "Yes", you will need to update processes or policies to avoid the repetition of the nonconformity.



If everything is correct and the actions are truly effective, you will be able to close the NCR.


Ineffective evaluation:


If it is identified that the problem persists or may reoccur, the "No" option must be selected. This decision indicates that the action was not sufficient to resolve the problem's cause, requiring a new approach. When closing the current NCR, the system will automatically open a new revision, ensuring the continuity of the treatment until the nonconformity is effectively resolved.

Evaluation of Risks and Opportunities: Even if the cause has not been eliminated, it is important to verify if the problem has raised the risk level or generated new risks that need to be mapped or adjusted. If the answer is "Yes", in the "Observation" field, objectively write details about the need: 


Changes in the Quality Management System: Next, it will be necessary to verify if the ineffectiveness of the actions required changes in the quality management system. If the answer is "Yes", it may be necessary to adjust processes or procedures to avoid the repetition of the problem in the future:

 


When clicking on Close NCR, the system will start the new revision to proceed with the treatment of the nonconformity, just click on View revision


Postpone the Effectiveness Evaluation


The "Postpone" option allows you to delay the effectiveness evaluation to a future date. This is useful when it is not yet possible to evaluate if the corrective actions were effective and you need more time or information.



To do this, just define a future date for the re-evaluation and insert who will be responsible for performing the evaluation when the deadline arrives and click Conclude.



The effectiveness pending task will be updated with the new chosen deadline.



The effectiveness evaluation ensures that the nonconformity was in fact resolved and that measures were taken to prevent its recurrence, strengthening the quality management system.
